PROGRAM NAME: JumpStart to Permanency GRADE: 23 OBJECTIVE:  To drive complex dependency cases toward permanency through assessment of cases, engagement of families, and engagement of the county provider network ESSENTIAL FUNCTIONS MAY INCLUDE: � Provide support and oversight for most complex dependency in-crisis or problem cases; provide ongoing assistance to program staff in coordinating services for clients � Conduct face-to-face interviews with various parties of the dependency case to develop a Systems Assessment � Meet individually and/or as a group with parties of the dependency case to facilitate the permanency process � Ensure the treatment/service team generates necessary reports and paperwork in a timely manner, confer and consult with DCM supervisors, providing advice and support � Liaison with community agencies such as ADM, Dept. of Children and Families, Department of Juvenile Justice, Community providers, etc. to ensure family supports are identified � Ensure that service delivery is performed according to and within the CHS mission, policy/statewide directive/procedural directions/treatment model, COA Standard, contractual requirements, and Florida Administrative Code � Provide modeling, perspective, empowerment and support which enables direct service personnel to deliver the service effectively � Participates in the quality management process This list of essential functions is not intended to be exhaustive.
